How do I register as a patient of Peak Vista?

There are two ways you can register as a patient of Peak Vista. You can call 719-632-5700 or walk in to the health center closest to you. If you have Medicaid, CHP+ , Medicare or Private Insurance – click here to complete the registration form!

How much does it cost?

Unemployed and uninsured individuals are welcome, as well as those with insurance, Medicaid or Medicare. If you do not have insurance, the fee for a visit will be based on the size of your family and your total income.

When will I get to make an appointment to see a physician/provider?

When you register, you will be asked to make your first medical appointment so you may get to know your physician/provider. There is no waiting for seniors, pregnant women or children . Some of our Centers have immediate appointments available. Others will be called as soon as possible. For all other appointments, call the center closest to you.

What if I am sick and need to see a physician/provider only one time?

Peak Vista’s Immediate Care Center is good for one visit only and does not cover other services such as lab and pharmacy. Call 632-5700 and our staff will set up a medical appointment and a one day registration. If you do not have insurance, the co-pay for the visit will be based on the size of your family and your total income.
